SPORTS PHYSICAL INFORMATION

• All  athletes  must  have  a  physical  form  filled  out  and  signed by  a  physician  on  file  at  school to  be  able  to  participate in  practice.

• Any  physical  prior  to  April  1, 2023  is  no  longer  valid  for  the  2024 / 2025 School  Year.

• Physicals  dated  from  April  1, 2023  to  March  31, 2024  require  you  to  fill  out  the  online  Alternate  Year  Card.

• Wellness  Check-Up  Forms  are  not  Acceptable
